COVID-19 Vaccines available for patients 6months and older

  • please call office to schedule all COVID-19 vaccines. 


  • Vaccines available: 
    • 6 months-5 years: Moderna 25 mcg  
    • 5 y/o-11 y/o: Pfizer 10 mcg
    • 12 y/o and older: Pfizer adult dose 
  • Do NOT give pain relievers/fever reducers (i.e. tylenol, motrin, acetaminophen, ibuprofen) PRIOR to the vaccine appointment. If your child experiences symptoms (injection site soreness, fever) AFTER the injection, it is OK to give pain/fever reducing medication.


  • If you/your child is sick, please cancel your child’s vaccine appointment.

  • print and complete and sign the following pre-vaccination checklist. Please REMIND US VERBALLY if your child has had a history of severe allergic reaction/anaphylaxis. If so, your child should be observed IN the office for 30 minutes after vaccine. 


When you arrive to the office:

  • Please enter the office only a few minutes PRIOR to your scheduled appointment time and remain in the waiting room until you are called. We want to minimize crowding of our waiting room. Masks required at all times.

What to do after the shot:

Following your child’s vaccine, you are required to wait for 15 minutes for observation. You will return to your car and wait in the parking lot for 15 minutes. You will keep track of the time. If there are any concerns with your child, please bring your child back into the office. Once 15 minutes has passed and if your child is feeling well, you may leave.
