Billing Info
- It is the patient’s responsibility to know their insurance benefits. Please contact your insurance company regarding coverage and participation.
- All payment (copays) is expected on the date of service. We accept cash, check, credit or debit for payment.
- All questions regarding your bill can be addressed with our billing staff Tuesday through Thursday from 9:00 am to 4:30 pm.
Referral Info
A referral is necessary for services provided outside our office for patients with an HMO insurance program.

- A visit to a specialist (ie. cardiology, dermatology, orthopedics etc)
- Physical therapy
- Occupational therapy
- Speech therapy
- Radiology procedures
- Emergency department visits
The office requires a five day prior notice for all necessary referrals.
